Industry Summit Delegate and First Time Next Generation Member
Discounted Registration: Includes the Motion Control
panel discussion and Showcase and Welcome Reception with open bar and
plentiful buffet on 10/21; Welcome Breakfast, Keynotes, Concurrent
Sessions, Annual Business Meeting with lunch, scheduled breaks and PTD A
50th Anniversary Celebration with open bar and dinner on 10/22;
breakfast and scheduled breaks on 10/23. Golf and tours are optional
events with separate registration fees.
To help grow distributor membership and build distributor attendance
at the Industry Summit, manufacturer members are encouraged to host a
non-member distributor to allow them to experience one of the most
effective benefits of membership. "Hosting" includes paying the full
non-member registration fee for the distributor delegate. Hosting
manufacturers will receive a discounted registration fee on their third
company delegate registration in support of this effort. To receive the
discount, choose the "Manufacturer Discount Registration" on the
registration form for the third delegate registered from your company
and identify the name of the distributor company you are hosting. The
completed registration form and payment for the non-member distributor
you are hosting must be submitted together with the registration form
for the third delegate. This discount is not available through online
registration.
Industry Summit Spouse/Companion Registration:
Spouses/companions may register a la carte for the Welcome Reception
(10/21), A Walk in the Desert event and PTD A 50th Anniversary
Celebration (10/22) or may select the all-inclusive registration option
and save 10%. Golf and tours are optional events with separate
registration fees. For information on fees, please see the registration
form on the back of this brochure. Spouses/companions who plan to attend
keynotes, concurrent sessions, the Motion Control Showcase and/or
MD-IDEX must register as delegates.
Golf Registration:The PTDA Foundation Golf Outing is
an optional event with separate registration fees. Golfers have the
opportunity to purchase a foursome for US $1,000. If you are interested
in purchasing a foursome, please list the confirmed members of your
foursome on your registration form. If you register as an individual,
you will not have the opportunity to choose your foursome (excluding
spouse/companion registrants). Due to overseeding of the course, the
Faldo Championship Course is cart-path only. If you have medical reasons
that require you to ride or have golf/sponsorship questions, please

Tour Registration: All tours are optional events
with separate registration fees to be paid directly to Hello
Arizona! Tours will be meeting in the hotel lobby at the published
start time. All tours must be reserved in advance--there will be
limited on-site registration. To register for the tours, complete the
separate tour registration form and mail or fax it with
your payment directly to Hello Arizona! by October 15, 2010.
MD-IDEX or Motion Control Showcase
Reservations:MD-IDEX booths and Motion Control Showcase tables
must be reserved separately; to reserve a booth or table, contact Brenda
Holt at +1.312.516.2100 or bholt@ptda.org. Only PTDA
manufacturer and associate members who have reserved an MD-IDEX booth
and all distributors will be allowed in the MD-IDEX session.
Companies reserving booth or table space also must register each
attending delegate for the Industry Summit.
Online Registration
To register online, visit www.ptda.org/SummitRegistration and
log in with your e-mail address and password. Note:
A separate login and password is required for each individual you wish
to register. For assistance logging in, contact PTDA at
+1.312.516.2100 or ptda@ptda.org. A credit card is
required for online registration.
Please note that registration discounts (First Time Next Generation
Member discounted registration and reduced registration for manufacturer
hosting a non-distributor member) are not available through online
registration. To take advantage of these discounts, review the
Registration Policies above and register by mail or fax.
Mail or Fax Registration
Please review the following instructions before registering:

A separate registration form must be received for each
delegate. To register additional individuals from your firm,
please download additional copies of the registration form as necessary.

Indicate spouse/companion participation in social events, if any,
by selecting the all-inclusive spouse/companion registration fee (Option
1) or adding the fee for each event you wish to attend (Option 2).

Indicate participation in the optional PTDA Foundation Golf Outing
by including the fees on the subtotal line. If purchasing a
foursome, include the names and company affiliations of those who have
been confirmed for your foursome.

Issue one payment covering all fees payable to PTDA in U.S.
funds. Your payment must accompany the registration form.

Advance Registration Deadline
To receive the early discounted registration fee, your registration
form and payment must be received no later than August 16, 2010. To
receive the regular registration fee and ensure inclusion in the advance
PTDA registration list, form and payment must be received no later than
September 15, 2010.
Refund Policies
Delegate and Spouse/Companion Registration:
Written cancellations received prior to September 15, 2010 will be
refunded in full minus a US$50 per person administrative fee. Due
to PTDA's contractual obligations with its vendors, no refunds can be
issued for cancellations received on or after September 15.
Money Back Guarantee: As a distributor
delegate to the Industry Summit, if you believe your attendance at the
Industry Summit was not a worthwhile experience and you did not walk
away with new ideas of value equivalent to the registration fee, PTDA
will refund your delegate registration fee. Contact PTDA following the
Industry Summit at +1.312.516.2100. The guarantee does not apply to
other meeting expenses such as hotel, transportation, air fare, etc.
Golf Outing: Written cancellations received
prior to September 15, 2010 will be refunded in full.
Cancellations received on or after September 15, including on-site
cancellations, will be refunded only if a substitute is found by the
registered guest to fill the space.
Tours: Please refer to the tour registration form for tour refund
policies.
Publicity Photo Acknowledgement
PTDA uses photographs of participants at the Industry Summit to
promote the association and its programs, publications and
services. Please be aware that these photos are for PTDA use only
and may be used in the association's publications and Web site. If you
do not wish to have your picture taken, please inform our
photographer.
Attire
Business casual attire is appropriate for all Industry Summit
activities. Resort wear is appropriate for the Welcome
Reception. Western attire is encouraged for the PTDA 50th
Anniversary Celebration. Casual attire is recommended for A Walk
in the Desert.
Private Meetings and Entertainment Policies
Private meetings are prohibited during all Industry Summit business
sessions, educational programs, MD-IDEX and social events. The
evening of Saturday, October 23 has been reserved for
manufacturer-sponsored hospitalities and receptions and may begin after
