Registration Information
Industry Summit Delegate Registration: The Delegate
Registration Fee includes the Welcome Reception with open bar and hors
d’oeuvres on 10/22; Welcome Breakfast, Opening Keynote Address,
Concurrent Workshops, Annual Business Meeting with lunch, Motion Control
Showcase and scheduled breaks on 10/23; breakfast, scheduled breaks,
Closing Keynote Address with lunch and the Closing Event with open bar
and dinner on 10/24. Golf, tours and the Motion Control Technical
Conference are optional events with separate registration fees.
Industry Summit Spouse/Companion Registration:
Spouses/companions may register a la carte for the Welcome Reception
(10/22), A Walk on the Bay event (10/23) and the Closing Event (10/24)
or may register for all of the above events and save 10%. Golf,
tours and the Motion Control Technical Conference are optional events
with separate registration fees. For information on fees, please
see the registration form. Spouses/companions who plan to attend
workshops, the Motion Control Showcase and/or MD-IDEX must register as
delegates.
Motion Control Technical Conference and Showcase
Registration: The Motion Control Technical Conference and
Showcase Registration Fee includes the Motion Control Technical
Conference program on 10/22 and 10/23 and breakfast and the Motion
Control Showcase on 10/23. Tickets to the Welcome Reception
(10/22) may be purchased a la carte. Attendees who plan to attend
concurrent workshops and/or MD-IDEX must register as delegates for the
Industry Summit.
Golf Registration: The Golf Outing is an optional
event with separate registration fees.
Tour Registration: All tours are optional events
with separate registration fees to be paid directly to PRA Destination
Management. Please download the registration form or register online at www.pra-tours.com/PTDAFPDA. All tours
must be reserved in advance—there will be no on-site
registration.
MD-IDEX or Motion Control Showcase Reservations:
MD-IDEX booths and Motion Control Showcase tables must be
reserved separately. To reserve a booth or table, contact
Brenda Holt at +1.312.516.2100 or bholt@ptda.org. Only PTDA
manufacturer and associate members who have reserved an MD-IDEX booth
and all distributors will be allowed in the MD-IDEX. Companies
reserving booth or table space also must register each attending
delegate for the Industry Summit.
Online Registration
To register online, visit www.ptda.org/SummitRegistration
and log in with your e-mail address and password. A separate login
is required for each registration. For assistance logging in,
contact PTDA at +1.312.516.2100 or ptda@ptda.org. A credit card is
required for online registration.
Mail and Fax Registration
Please review the following instructions before registering:

A separate registration form must be received for each
delegate. To register additional individuals from your firm,
please download additional copies of the registration form as necessary or contact PTDA for
additional forms (+1.312.516.2100; ptda@ptda.org ).

Indicate registration for the Motion Control Technical Conference
by including the fee on the subtotal line.

Indicate spouse/companion participation in social events, if any,
by adding the fee for each event (Option 2) or selecting the
all-inclusive guest registration fee (Option 1).

Indicate participation in the optional Golf Outing by include the
fees on the subtotal line.

Issue one payment covering all fees payable to PTDA in U.S.
funds. Your payment must accompany the registration form.

Advance Registration Deadline
To receive the early discounted registration fee and ensure inclusion
in the advance PTDA registration list, your registration form and
payment must be received no later than September 15, 2009.
Refund Policies
Delegate, Spouse/Companion and Motion Control Technical
Conference Registration: Written cancellations received
prior to September 30, 2009 will be refunded in full minus a US$50 per
person administrative fee. Due to PTDA’s contractual
obligations with its vendors, no refunds can be issued for cancellations
received on or after September 30.
Golf Outing: Written cancellations received prior to
September 30, 2009 will be refunded in full. Cancellations
received on or after September 30, including on-site cancellations, will
be refunded only if a substitute is found by the registered guest to
fill the space.
Tours: Please refer to the tour registration form for tour refund
policies.
Publicity Photo Acknowledgement
PTDA uses photographs of participants at the Industry Summit to
promote the association and its programs, publications and
services. Please be aware that these photos are for PTDA use only
and may be used in the association’s publications and Web site. If
you do not wish to have your picture taken, please inform our
photographer.
Attire
Business casual attire is appropriate for all Industry Summit
activities. Casual attire is recommended for A Walk on the
Bay on Friday, October 23. Flat-soled shoes and casual attire are
recommended for the closing event on the USS Midway due to the grated
surfaces and the use of ladders to navigate different areas of the
ship.
Private Meeting and Entertainment
Policies
Private meeting are prohibited during all Industry Summit business
sessions, educational programs, MD-IDEX and social events. The
evening of Friday, October 23 has been reserved for on-site
manufacturer-sponsored hospitalities and receptions and may begin after
7:00 p.m. Download PTDA’s complete entertainment
